The FMUSP-HC Academic Health System provides complete assistance at all levels of health care. Primary care is carried out by the Butantã School Health Center and Basic Health Units, medium complexity care (secondary care) is carried out by the Universitary Hospital of USP and high complexity procedures and care (tertiary care) are carried out by the various institutes and units of the Hospital das Clínicas of FMUSP (HCFMUSP). The System also has specialized centers such as the Extension Service for the Care of HIV/AIDS Patients and the Children's Cancer Treatment Institute (ITACI).

Since 1988, the FFM has been responsible for the agreement signed between the HCFMUSP and the São Paulo State Department of Health, which provides free medical assistance to patients of the “Sistema Único de Saúde” - SUS (Unified Health System). SUS services at HCFMUSP (except InCor) are ensured through the allocation of human and financial resources from the System at the Hospital itself and reach approximately 90% of all hospitalizations and outpatient procedures performed in the complex. The agreement also guarantees the performance of transplants, various implants, highly complex procedures and comprehensive pharmaceutical assistance.

Currently, the Foundation employs almost 12,000 employees, of which more than 90% are dedicated to the SUS patients care. It also invests resources in the infrastructure of the complex through the acquisition of medicines, inputs, equipment and furniture, among others.

In addition, and since 2008, the FFM is responsible for managing the Cancer Institute of the state of São Paulo (Icesp) and the Lucy Montoro Rehabilitation Institute (IRLM), and in 2022, for Perdizes Institute (IPer).
